Every reasonable attempt is made for accuracy, but some information specified in or accessible through the Sites is compiled or produced by someone other than and beyond the control of our staff and may not be fully complete or accurate. Consequently, we cannot and do not endorse or represent the reliability or accuracy of all of the content or information distributed through or accessed from the Sites. You hereby agree that your reliance upon any content or information distributed through or accessed from the Sites is at your sole risk.

These terms and conditions apply only to our Sites, and not to the sites of any other companies or organizations, including those to which any of the Sites may link. We are not responsible for the availability of any other site to which any of the Sites links. We do not endorse or take responsibility for the contents, advertising, products or other materials made available through any other site. The information contained in these Sites or transmitted to you electronically in response to a message from you is not intended nor implied to constitute medical advice, diagnosis, or treatment. Always visit or speak to a qualified health service provider in person prior to starting any new treatment or with any questions you may have regarding a medical condition. These Sites do not directly or indirectly practice medicine or dispense medical advice as part of this service. In addition, nothing contained in these Sites is intended to be or constitutes legal advice, and users should always seek the advice of an appropriate attorney or other professional regarding individual questions or concerns of a legal or professional nature. We assume no liability for any diagnosis, treatment, decision made, or action taken in reliance upon information contained in these Sites or any other internet sites linked to it in any way.
PARENTAL PERMISSION
Some of the information available on the Sites concerns the topic of sex and may not be appropriate for children. Further, the Sites are not directed to children under the age of 13 and we will not knowingly collect personally identifiable information from children under 13. We strongly recommend that parents participate in their children’s exploration of the internet and any online services and use their browser’s parental controls to limit the areas of the internet to which their children have access.
UNIFORM DISCLOSURE STATEMENT
Our organization is recognized by the IRS as a ministry exempt from taxation under Section501(c)(3) of the Internal Revenue Service Code, and gifts are tax deductible to the extent allowed by law.
State statutes and regulations regarding charitable donations may differ from Federal statutes and regulations covering the same subject matter or transaction. State statutes and regulations may vary from state to state. In order to verify your own state’s statutes or regulations or to validate the tax deductible status of your charitable donation within your state, it is your sole and personal responsibility to submit your inquiry to the appropriate governing agency in your state and to consult legal counsel at your own effort and expense.
HEALTH INFORMATION & NOTICE OF PRACTICES
At this point in your care, no specific treatment plan has been recommended nor are you under the supervision of our medical providers. This consent form is simply an effort to obtain your permission to perform the evaluation necessary to identify the appropriate treatment and/or procedure for any identified condition(s).
Aspire Medical Services and Education endorses, supports, and participates in electronic Health Information Exchange (HIE) to improve the quality of your health and healthcare experience. HIE provides us with a way to securely and efficiently share patients’ clinical information electronically with other physicians and health care providers that participate in the HIE network. Using HIE helps your health care providers to more effectively share information and provide better care. The HIE also enables emergency medical personnel and other providers who are treating you to have immediate access to your medical data that may be critical for your care. Making your health information available to your health care providers through the HIE can also help reduce your costs by eliminating unnecessary duplication of tests and procedures. However, you can choose to opt out of the Patient Information sharing in the HIE system any time.
The consent provides us with your permission to perform reasonable and necessary medical examinations, testing and treatment. By signing below, you are indicating that (1) you intend that this content is continuing in nature even after a specific diagnosis has been made and treatment recommended; and (2) you consent to treatment at this office. The consent will remain fully effective until it is revoked in writing. You have the right at any time to discontinue services.
You have the right to discuss the treatment plan with your physician about the purpose, potential risks, and benefits of any test ordered for you. If you have any concerns regarding any test or treatment recommended by your healthcare provider, we encourage you to ask questions.
I voluntarily request a physician, and/or mid-level provider (Nurse Practitioner, Certified Midwife, Physician Assistant) and other healthcare providers, or designees as deemed necessary, to perform reasonable and necessary medical examination, testing and treatment for the condition which has brought me to seek care at this practice. I understand that if additional testing, invasive or interventional procedures are recommended, I will be asked to read and sign additional consent forms prior to the test(s) or procedure(s).
NOTICE OF PRIVACY PRACTICES
Our Notice of Privacy Practices provides information about how we may use and disclose protected health information about you.
- I consent to the use or disclosure of my protected health information by the medical providers at Aspire Medical Services for diagnosing or providing treatment to me, obtaining payment from insurance companies or to conduct health care operations of the practice.
- I understand I have the right to request a restriction as to how my protected health information is used or disclosed to carry out treatment, payment or healthcare operations of Aspire.
- My “protected health information” means health information, including my demographic information, collected from me and created or received by my physician, another health care provider, a health plan, my employer or a health care clearinghouse. This protected health information relates to my past, present or future physical or mental health or condition and identifies me, or there is a reasonable basis to believe the information may identify me.
- I hereby authorize and assign all payment and/or insurance benefits for medical services and/or procedures to Aspire. I understand that I am responsible for all charges not covered by my insurance plan including, but not limited to, copays and deductibles.
LIMITATION OF SERVICES
This facility is a nonprofit charitable organization. Services are provided by medical staff and trained client advocates.
The urine pregnancy test is 97-99% accurate; however a physician should confirm the results of your test. Performing a pregnancy test and/or early ultrasound does not automatically establish you as a patient under the care of the Aspire physician on staff. Care with our physician is established during the first doctor appointment around 10-12 weeks of your pregnancy.
We provide limited ultrasounds to confirm intrauterine pregnancies, dates, and viability. Ultrasound should not be substituted for prenatal care. Pregnant clients are advised to seek prenatal care.
Our sexually transmitted disease services are limited to diagnosis and treatment of Chlamydia and Gonorrhea. We comply with Colorado state mandated reporting of positive results.
Our advocates are lay people, not necessarily licensed or degreed personnel. We do provide counseling services with licensed professional counselors (LPCs) through Authentic Solutions at Aspire. They adhere to strict privacy and confidentiality policies with each client and do not share information from counseling sessions with the Aspire staff, unless the client authorizes the counselor to share information.
All information is kept confidential unless reporting laws regarding abuse or neglect apply, or if we believe or hear that you are in danger of hurting yourself or others.
For the privacy of our clients, our center and its agents do not consent to any recording on the premises, including recording of ultrasounds.
We do not perform or refer for abortion, but we do provide information and services so that you are equipped to make an informed choice about your pregnancy or diagnosis.
Our policy is that the CLIENT ONLY meets with the volunteer/staff during the initial part of her visit. This is to protect client confidentiality. Others will be invited in at the client’s request later in the appointment. Please be advised that appointments typically last 45-60 minutes. If this is inconvenient for you, let the staff know and an appointment will be scheduled at a more convenient time.
